Enrollment Contracts

Once placement at Fusion Academy is deemed appropriate for your child by both you and Fusion administration, please complete and return the appropriate enrollment contract along with a contact and deposit form in order to officially enroll your child in our program. The completed forms may be returned to us either in person, by mail, or by fax.
